To Make the box,
Cut your cardstock 7" x 12"
Measure using the Scor-Pal or Fiskars Scoring blade with the 12" side to (horizontally)
2", 2 1/2", 7", 7 1/2"
(Your first section will be 2", then you will have a 1/2 " section, then 4 1/2", 1/2 section, then 4 1/2" section. ) See below
Now, turn your cardstock to the 7" side at the top horizontally, and score 1/2 on either side.
Cut triangles at the flap areas to make the box close together on the sides, and it leaves room for ease.
In the top section (2" side) punch a hole 1" from the edge, 3" from the side

In the top section (2" side) punch a hole 1" from the edge, 3" from the side
Punch the hole in this section, approx 2 1/2 " down from the edge, and in the center (3 1/2")from the side. This is the 4 1/2 " section.
Adhere tape to the side flap to assemble the box. USE STICKY tape or some tape that will adhere well. MonoAdhesive (or SNAIL) is not advised because it will lose it's stickiness after a short time or in humid weather.

This is great! Wish I would have seen this yesterday...I gave a set of cards away in a boring cello bag when I could have made a beautiful gift box like this. There's always next time! TFS!
